UK Teen’s Mass Stabbing at Taylor Swift-Themed Dance Class “Could Have Been Prevented,” Report Finds

A devastating attack that claimed the lives of three young girls at a Taylor Swift-themed dance class in northwestern England “could and should have been prevented,” according to a comprehensive report released Monday by retired judge Adrian Fulford.

The 763-page document details numerous missed opportunities when parents and authorities could have intervened to stop 17-year-old Axel Rudakubana before he fatally stabbed three children and wounded 10 others in Southport in 2024.

“One of the most striking conclusions from this inquiry’s extensive investigation is the sheer number of missed opportunities over many years to intervene meaningfully, which directly contributed to the failure to avert this disaster,” Fulford stated. “The consequences were catastrophic.”

Rudakubana is currently serving a life sentence with no possibility of parole for 52 years for killing Alice da Silva Aguiar, 9, Elsie Dot Stancombe, 7, and Bebe King, 6, in what Fulford described as an attack of “extreme and very particular depravity” unprecedented in the UK.

The incident sparked days of civil unrest after far-right groups circulated false information claiming the perpetrator was a Muslim migrant. In reality, Rudakubana was born in Wales to Rwandan Christian parents.

The inquiry revealed an alarming pattern of warning signs that went unaddressed by various authorities. At age 13, Rudakubana was convicted of assaulting another child with a hockey stick. Between 2019 and 2021, he was referred to the government’s anti-extremism program, Prevent, three times for expressing interest in school shootings, terrorist attacks, and extremist organizations. Each time, his case was closed because evaluators didn’t consider him susceptible to terrorism.

During this period, police were called to his home five times over behavioral concerns. Despite receiving mental health support and educational assistance, Rudakubana eventually stopped engaging with social workers. After being expelled for bringing a knife to school, he rarely attended his subsequent educational placement.

“Far too often, AR’s ‘case’ was passed from one public sector agency to another in an inappropriate merry-go-round of referrals, assessments, case-closures and ‘hand-offs,'” Fulford wrote, using only the killer’s initials in the report.

The inquiry highlighted a particularly critical incident in March 2022 when Rudakubana was caught on a bus with a knife and told police he wanted to stab someone. He also admitted attempting to make poison. Fulford stated that these admissions should have prompted an arrest, which likely would have led to a search of his home, revealing his purchase of seeds to make ricin and downloaded terrorist materials on his computer.

Instead, Rudakubana was released to his parents, who reportedly feared their son but repeatedly failed to report his knife purchases, disturbing behavior, and explicit threats. While acknowledging these parental failures, Fulford cautioned against vilifying them, noting that their home life had become “little short of a nightmare” as their son transformed into what his father described as a “monster.”

Following the Southport attack, police discovered ricin hidden under Rudakubana’s bed along with an al-Qaida training manual. However, authorities determined his crimes didn’t qualify as terrorism because he lacked a clear political or religious motivation.

UK Prime Minister Keir Starmer called the report “truly harrowing and profoundly disturbing” and promised fundamental changes to address the systematic failures identified. “While nothing will ever bring these three little girls back, I’m determined to make the fundamental changes needed to keep the public safe,” Starmer said.

Home Secretary Shabana Mahmood announced plans for new legislation to address violent plots that don’t meet the current definition of terrorism. “Unlike terrorist attacks, if you are planning an attack without an underlying ideology, there is no crime on the statute book,” Mahmood explained.

The report issued 67 recommendations aimed at preventing similar tragedies in the future, focusing on improved coordination between agencies, better risk assessment protocols, and clearer pathways for intervention when concerning behaviors emerge.
