Cruise Ship Outbreak Claims Three Lives Amid Delayed Evacuations, Hantavirus Investigation

A suspected hantavirus outbreak aboard a cruise ship in the Atlantic Ocean has claimed three lives and left several others ill, according to the World Health Organization. The UN health agency confirmed one case through laboratory testing, with five additional suspected cases awaiting confirmation.

Among the six affected individuals, three have died and one is currently receiving intensive care in South Africa. The WHO is coordinating with governments and the vessel’s operator to arrange medical evacuation for two symptomatic passengers still aboard the ship.

“Detailed investigations are ongoing, including further laboratory testing, and epidemiological investigations,” the WHO stated. “Medical care and support are being provided to passengers and crew. Sequencing of the virus is also ongoing.”

The outbreak has been traced to the m/v Hondius, a Dutch-flagged cruise vessel currently sailing off the coast of West Africa. Oceanwide Expeditions, the company operating the ship, confirmed the three passenger fatalities and acknowledged that one passenger is under intensive care in Johannesburg. Additionally, two crew members aboard the vessel require urgent medical attention.

As of Sunday evening, authorities in Cape Verde had not authorized the disembarkation of passengers needing medical treatment or broader health screening. While local health officials have boarded the vessel to assess the situation, they have yet to approve the transfer of symptomatic individuals to land-based medical facilities.

“The priority of Oceanwide Expeditions is to ensure that the two symptomatic individuals on board receive adequate and expedited medical care,” the company emphasized in a statement.

Dutch authorities are working to coordinate repatriation efforts for affected individuals from Cape Verde to the Netherlands, though these plans remain contingent on approval from Cape Verdean officials.

Hantavirus infections typically occur through exposure to urine or feces from infected rodents. The disease can progress to severe respiratory illness with potentially fatal outcomes. Although person-to-person transmission is rare, the WHO noted that it can occur and requires careful monitoring and medical support.

“While rare, hantavirus may spread between people, and can lead to severe respiratory illness and requires careful patient monitoring, support and response,” according to the WHO statement.

The emergence of hantavirus on a cruise ship represents an unusual outbreak scenario, as the virus is typically associated with exposure to rodents in rural or wilderness settings. Cruise ships implement strict sanitation protocols specifically designed to prevent rodent infestations, raising questions about how the virus may have been introduced to the vessel.

There is no specific antiviral treatment for hantavirus infection, though early medical intervention and supportive care can significantly improve survival rates. Symptoms typically include fever, muscle aches, and severe respiratory problems that can rapidly progress to respiratory failure.

The WHO has notified global health authorities under international health regulations and continues to support response efforts. The agency is working to assess the public health risk to others who remain on board the vessel.

“We are currently establishing the full facts and working on appropriate medical care, screening, and next steps,” Oceanwide Expeditions said in its statement.

This outbreak follows several high-profile cases of infectious disease transmission aboard cruise ships in recent years, highlighting the challenges of disease containment in the confined environments of passenger vessels. Maritime health experts note that cruise ships present unique challenges for infection control due to the concentration of people in enclosed spaces with shared ventilation systems.
