President Trump’s administration deployed a government plane to Cuba this week to retrieve a 10-year-old Utah child caught in a contentious custody dispute involving questions of gender identity.

The child’s parent, Rose Inessa-Ethington, a transgender woman, and her partner Blue Inessa-Ethington were arrested and charged with international parental kidnapping after allegedly taking the child to Cuba without the biological mother’s permission.

Federal authorities initiated the search after family members expressed concerns that the couple had traveled to Cuba seeking gender transition surgery for the child. According to court documents filed Monday in federal court in Utah, the pair had initially told the biological mother they were taking the child camping in Canada in late March, along with Blue’s 3-year-old child.

After informing the mother they had arrived in Canada, the couple turned off their phones and instead flew from Vancouver to Mexico and then to Cuba on April 1, investigators said.

The FBI investigation revealed Blue Inessa-Ethington withdrew $10,000 from her checking account before departing. Agents later discovered a note at the couple’s home containing instructions from a Washington, D.C. mental health therapist regarding gender-affirming medical care for children and directions to send the therapist $10,000. The note did not specifically mention Cuba.

Law enforcement officials noted that gender-affirming surgery for children is not legal in Cuba. In fact, Cuban regulations only permit such procedures for adults who are Cuban citizens, following a comprehensive medical review process that typically takes years and requires numerous evaluations.

The deployment of a Department of Justice plane for this parental kidnapping case comes amid broader tensions over transgender healthcare access. The Trump administration had previously moved to restrict gender-affirming care for minors and pressured healthcare providers on the issue.

Logan City Police Chief Jeff Simmons explained that authorities initially focused on custodial interference allegations when the child wasn’t returned to the biological mother as scheduled on April 3. The mother, who shared custody with Rose Inessa-Ethington following their divorce, filed a missing-person report with police in Logan, Utah.

Concerns about potential gender-affirming surgery emerged later in the investigation, according to police spokesperson Sgt. Brandon Bevan. “They just had the concern about it, no actual physical evidence,” Bevan said, noting that the concerns were raised by one family member.

On April 13, a Utah state judge ordered the child’s return to the biological mother. Three days later, a federal magistrate judge issued arrest warrants for the Inessa-Ethingtons. Cuban authorities located the group that same day.

The couple was deported to the United States aboard the government plane on Monday and arraigned in federal court in Richmond, Virginia. First Assistant U.S. Attorney Melissa Holyoak confirmed the 10-year-old has been returned to the biological mother. Officials declined to comment on the status of Blue’s 3-year-old child who had accompanied the group.

Court records suggest this custody dispute has been ongoing for years. Five years ago, Blue Inessa-Ethington created an online fundraiser titled “Help a Trans Mother Keep Custody of Her Child,” which raised nearly $10,000. The fundraiser claimed Rose’s ex had relocated “several counties away, negatively impacting Rose’s parent-time with the child.”

According to an FBI affidavit, family members stated the child was assigned male at birth but identifies as a girl, which they attributed to “manipulation” by Rose Inessa-Ethington.

The case intersects with broader national debates around gender-affirming care for minors. Research shows such medical interventions are rare among U.S. children. Major medical organizations advocate case-by-case decision-making, with fewer than 1 in 1,000 U.S. adolescents receiving gender-affirming medications such as hormones or puberty blockers.

In December, the Trump administration moved to restrict gender-affirming care for minors, prompting legal challenges from numerous states. The policy reflected ongoing tensions between an administration that has characterized transgender healthcare as potentially harmful to children and advocates who maintain these treatments are medically necessary for some youth.
