Federal Judge Temporarily Blocks Trump Administration’s Health Grant Cuts to Democratic States

A federal judge in Illinois has temporarily halted the Trump administration’s plan to rescind $600 million in public health grants earmarked for four Democratic-led states, ruling that the cuts could cause “irreparable harm” to essential health programs.

U.S. District Judge Manish Shah issued a 14-day pause on Thursday, preventing the Centers for Disease Control and Prevention (CDC) from withdrawing funding that supports critical public health initiatives in California, Colorado, Illinois, and Minnesota. The ruling came just one day after the states filed a lawsuit challenging the planned cuts.

“The states have shown that they would suffer irreparable harm from the agency action,” Shah wrote in his order, ensuring that grant money will continue flowing to state and local health departments while the legal challenge progresses.

Colorado Attorney General Phil Weiser noted the ruling came just in time, as the first batch of grants could have been pulled that same day if the judge had not intervened.

The contested funding primarily supports programs that track disease outbreaks and study health outcomes of LGBTQ+ communities and people of color in major urban centers. A significant portion of the money helps cities combat the spread of HIV and other sexually transmitted infections among vulnerable populations, including gay and bisexual men, adolescents, and ethnic minorities.

The Department of Health and Human Services justified the termination by claiming the grants no longer align with CDC priorities, which were revised last year. These revisions reflect the administration’s shift away from health equity policies—approaches that provide additional support to certain populations to eliminate health disparities.

Officials from the four states have characterized the cuts as politically motivated retaliation for their opposition to the administration’s immigration enforcement policies. All four states have previously been targets of other federal funding reductions affecting food assistance programs, child care subsidies, and electric vehicle infrastructure.

Illinois Attorney General Kwame Raoul, who is leading the lawsuit, argued that the health care cuts violate the Constitution by retroactively imposing conditions on funding that Congress had already approved.

“Targeting four Democrat-run states that are standing up to his completely unrelated immigration policies is a transparent attempt to bully us into compliance,” Raoul said. “The president may be playing politics with critical public health funding, including more than $100 million to Illinois, but our residents are the ones who pay the price.”

State officials warned that losing the funding would force them to lay off hundreds of public health workers, severely impacting their ability to monitor and respond to disease outbreaks and provide essential health services to vulnerable communities.

Minnesota Attorney General Keith Ellison indicated that the states will seek to extend the judge’s temporary pause for the duration of the lawsuit, which could potentially preserve the funding through a lengthy court process.

The legal battle represents another chapter in ongoing tensions between the Trump administration and Democratic-led states. Courts have previously blocked similar efforts by the administration to withhold federal funding, including a recent plan to cut billions for child care subsidies and other programs benefiting low-income families in the four states plus New York.

Public health experts have expressed concern that politicizing health funding could undermine decades of progress in disease prevention and health equity initiatives. The programs at risk have been credited with reducing HIV transmission rates and improving health outcomes in communities that historically face barriers to quality healthcare.

Federal health officials did not immediately respond to requests for comment on the judge’s order. The case is expected to move forward as states seek more permanent protection for the disputed funding.
