Iran’s New Supreme Leader Severely Disfigured Following US-Israeli Airstrikes

Iran’s new supreme leader Mojtaba Khamenei is reportedly recovering from severe leg and facial injuries sustained during joint US-Israel airstrikes on Tehran on February 28, according to a Reuters report published Saturday. The attack, which killed his father, former supreme leader Ayatollah Ali Hosseini Khamenei, has left the younger Khamenei significantly disfigured.

Three anonymous sources from Khamenei’s inner circle told Reuters that despite his injuries, the new supreme leader remains “mentally sharp” and continues to direct Iran’s affairs from seclusion. He has not made any public appearances since being hospitalized following the strikes.

The report aligns with earlier statements from US Secretary of War Pete Hegseth, who first mentioned on March 13 that Khamenei was “likely disfigured.” Hegseth reiterated this assessment during a Pentagon press briefing on Thursday while detailing the extensive damage inflicted on Iran’s leadership structure.

“Their top leadership was systematically eliminated,” Hegseth said, before listing a series of high-ranking Iranian officials killed in the operations. “The supreme leader military office chief dead, the defense minister no longer with us, the IRGC commander dead, the armed forces general staff commander dead, the intelligence minister dead, the IRGC navy commander no longer here, the IRGC Intel chief dead.”

Despite his compromised physical condition, Khamenei issued a defiant written statement on Thursday, declaring: “Iran is not seeking war but will not forfeit its rights and considers all resistance fronts as a unified entity.” Hegseth dismissed the statement as “weak” and questioned why Iran’s leader would communicate in writing rather than through video or audio.

“Iran has plenty of cameras and plenty of voice recorders. Why a written statement?” Hegseth asked rhetorically. “I think you know why. His father, dead. He’s scared, he’s injured, he’s on the run and he lacks legitimacy. It’s a mess for them. Who’s in charge? Iran may not even know.”

The leadership crisis comes at a critical time as Iran engages in peace negotiations with the United States. An Iranian delegation is currently meeting with American officials in Pakistan, where Vice President JD Vance is leading the US team. Sources indicate that Khamenei remains in communication with the Iranian representatives during these talks.

According to Reuters, Khamenei may attempt to make a public appearance within the next month or two, but only if “his health and the security situation allowed.” The uncertainty surrounding his physical condition has fueled speculation about the stability of Iran’s leadership and the regime’s ability to maintain control.

Iranian hardliners have defended Khamenei’s absence from public view as a security measure. “Why should he appear in public? To become a target for these criminals?” an Iranian militiaman asked Reuters in a text message, highlighting ongoing concerns about further attacks.

The February airstrikes represent the most significant blow to Iran’s leadership structure in decades, eliminating numerous high-ranking military and government officials in a coordinated operation. The succession of Mojtaba Khamenei occurred amid this unprecedented leadership vacuum, raising questions about the regime’s resilience and future direction.

Regional analysts note that the physical incapacitation of Iran’s supreme leader, combined with the elimination of much of the country’s security apparatus leadership, has created a power dynamic that strongly favors the United States and Israel in ongoing negotiations, potentially explaining Iran’s newfound willingness to engage in peace talks despite its traditionally defiant stance.
