In a significant development that challenges the narrative surrounding civilian casualties in Gaza, new research has revealed that many individuals previously counted as civilians were actually members of terrorist organizations like Hamas and Palestinian Islamic Jihad (PIJ). These findings could substantially alter the understanding of Israeli military operations and accusations of targeting civilians.
Gabriel Epstein, senior policy associate at Israel Policy Forum, has documented multiple cases of individuals who maintained public roles in healthcare or media while secretly serving as members of terrorist groups. When killed by Israeli forces, these individuals were mourned by their respective militant organizations as “martyrs” killed in battle.
One of the most notable cases involves Fadi al-Wadiyya, a physiotherapist for Médecins sans frontières (MSF), who was killed by Israel Defense Forces in June 2024. MSF initially expressed outrage, strongly condemning his killing. However, a Telegram account claiming to be the media reserve for PIJ’s Al-Quds Brigades later posted a memorial that identified al-Wadiyya as an assistant to the military manufacturing unit.
When questioned about this revelation, an MSF spokesperson stated, “We would not knowingly employ people engaging in military activity,” adding that such involvement “would pose a danger to our staff and patients by compromising our neutrality.” The organization claimed they had no indication of al-Wadiyya’s militant activities prior to Israeli authorities’ posts in June 2024.
The IDF had banned MSF operations in Gaza beginning in March because the organization refused to provide a list of its Palestinian employees. MSF defended this decision, explaining they did not receive “concrete assurances to ensure the safety of our staff” and cited concerns about humanitarian workers being “detained, attacked, and killed.”
Epstein’s research uncovered several other healthcare workers with dual roles. Mohammed Akram Abdullah al-Kafarna, who was publicly known as the nursing supervisor at Kamal Adwan Hospital and head of the Gaza nursing system, was identified by a Hamas-affiliated Telegram account as one of the “Qassam Martyrs” from the group’s elite Izz al-Din al-Qassam Brigades.
Similarly, Ayman Suleiman Aliyan Abu Tayr, listed as a nurse and head of clinical nutrition at Nasser Hospital, was reportedly a Commander in the Central Operations Unit of PIJ’s Al-Quds Brigades. Jaber Abdulhamid Diab Mohammedin, an ICU nurse at Al-Rantisi Specialized Children’s Hospital, was mourned by Islamic Jihad as a commander in their military manufacturing unit.
The pattern extends to media workers as well. Salo Aizenberg, director of media watchdog group HonestReporting, has tracked at least ten “virtually indisputable” examples of journalists who were actually combatants working with terrorist groups. These include Yacoup Al-Borsch, cited by the Committee to Protect Journalists (CPJ) as the executive director of Namaa Radio, but identified in multiple social media posts as a fighter and “mujahid.”
David Adesnik, vice president of research for the Foundation for Defense of Democracies, noted that the evidence indicates “this duplicity was a routine part of a strategy to infiltrate civilian organizations, especially humanitarian ones.” He explained that such infiltration “provides access and protection while ensuring outrage when these supposed humanitarians are killed.”
When asked about updating their listings of journalists who have been claimed as terror affiliates, the CPJ directed inquiries to their policy, which states they update data without issuing formal corrections as new information becomes available.
As the tenuous ceasefire in Gaza continues, analysts emphasize the importance of examining the war’s casualties. Epstein noted that “reviewing cases of militants who held dual civilian roles in key sectors like media, healthcare and education is important for the historical record” and helps reveal “just how deep Hamas, PIJ and other militant groups’ hold over key sectors in Gaza was.”
These findings may significantly impact international perceptions of the conflict and accusations against Israeli forces, highlighting the complex reality of distinguishing between civilians and combatants in urban warfare zones where terrorist groups deliberately blur these lines.
