A major healthcare fraud scheme targeting Ayushman Bharat, India’s flagship health insurance program, has been uncovered in Assam’s Hailakandi district. The Enforcement Directorate’s (ED) investigation revealed that operators of Shifa Hospital and Research Center orchestrated an elaborate scam that defrauded the government of over ₹64 lakh through hundreds of fictitious medical claims.
According to ED officials, the hospital’s operators recruited Ayushman Bharat beneficiaries by offering them ₹300 each to pose as patients. These individuals were photographed lying on hospital beds in a temporary facility, despite receiving no actual medical treatment. The images were then uploaded to the government’s Transaction Management System portal to generate reimbursement claims for non-existent medical procedures.
“This was a meticulously planned operation designed specifically to exploit vulnerabilities in the claim verification process,” said an ED official familiar with the investigation. “Our team found no evidence that Shifa Hospital ever functioned as a legitimate medical facility.”
Between April 2019 and November 2022, the fraudulent operation submitted 778 fake claims, resulting in the Atal Amrit Abhiyan Society—Assam’s state health agency—crediting approximately ₹57.96 lakh to the hospital’s bank account. Financial records show the funds were systematically withdrawn through cash transactions and UPI transfers.
The investigation began following an FIR filed at Hailakandi Police Station. When ED officials visited the hospital’s registered address, they discovered no operational healthcare facility existed at the location, confirming suspicions of fraud.
The Ayushman Bharat Pradhan Mantri Jan Arogya Yojana (AB-PMJAY), launched in 2018, provides health coverage of up to ₹5 lakh per family annually to economically vulnerable households across India. The program has been instrumental in providing healthcare access to millions, but experts have long warned about potential vulnerabilities to fraud.

Investigators determined that the accused had no legitimate source of income to justify these property acquisitions. In addition to the five land plots, they had constructed two multi-story buildings using the proceeds from the fraudulent scheme.
The case has raised concerns about oversight mechanisms within the national health insurance program. Health policy experts emphasize that while such schemes are vital for expanding healthcare access, they require robust monitoring systems and regular audits to prevent misuse.
