Government Shutdown Fueled by Healthcare Disputes Between Democrats and Republicans

The federal government has entered its first shutdown since 2019 as Democrats and Republicans remain locked in a standoff over healthcare provisions in a funding bill. At the center of the dispute are conflicting claims about healthcare premiums and immigrant coverage that have become political rallying points for both parties.

Democrats have consistently warned that healthcare premiums will increase by 75% without congressional action. This figure, which requires context, refers to the average increase in out-of-pocket costs for Americans receiving Affordable Care Act (ACA) subsidies if enhanced benefits expire at year’s end.

Meanwhile, Republican leaders have claimed that “Democrats demand we fund healthcare for illegal aliens.” This assertion mischaracterizes Democratic proposals, which seek to restore healthcare for “lawfully present” immigrants – not those living in the country illegally, who are prohibited by federal law from receiving federally funded coverage.

The impasse revolves around Democratic efforts to include an extension of ACA subsidies in the government funding legislation and to repeal certain Medicaid measures from the One Big Beautiful Bill Act. Republicans have rejected this approach, arguing that subsidy extensions could be addressed separately after passing a clean funding extension.

The 75% premium increase cited by Democrats comes from estimates by KFF, a health policy research organization, which found that out-of-pocket premium costs would rise significantly for subsidized marketplace enrollees if the enhanced tax credits expire. According to KFF, the average annual premium contribution for subsidized consumers would jump from $888 to $1,593 – a 79% increase. KFF has since updated their analysis, projecting an even higher 114% increase by 2026 due to rising premiums and Trump administration policy changes.

These enhanced ACA subsidies originated in the American Rescue Plan of 2021 during the COVID-19 pandemic. Initially set for two years, they were extended through 2024. The enhancements have proven effective, with marketplace enrollment more than doubling from 11.4 million in 2020 to 24.3 million in 2025. Currently, 92% of enrollees receive subsidies.

The Congressional Budget Office estimates that allowing these enhanced subsidies to expire would lead to 4.2 million more uninsured Americans by 2034.

Senate Majority Leader John Thune has suggested discussing the subsidy issue later, noting, “That doesn’t happen until the end of the year. We are going to have to have reforms if we take action there, but I think there is potentially a path forward.”

On the immigrant healthcare front, Democrats have proposed reversing provisions in the One Big Beautiful Bill Act that affected “lawfully present” immigrants – a classification that includes refugees, asylum seekers, and certain victims of domestic abuse or human trafficking.

House Minority Leader Hakeem Jeffries called the Republican characterization “an outright lie,” stating, “Federal law prohibits the use of taxpayer dollars to provide medical coverage to undocumented individuals.”

The Republican tax and budget law signed in July capped emergency Medicaid reimbursements for care provided to immigrants and changed Medicaid enrollment criteria to exclude those granted asylum and parolees. The Congressional Budget Office estimated these restrictions would result in 1.4 million lawfully present immigrants becoming uninsured, reducing federal spending by $131 billion by 2034.

As government agencies remain shuttered, the healthcare dispute continues to divide lawmakers, leaving thousands of federal workers furloughed and essential services curtailed until a funding agreement can be reached.
